									<h3><b>The current state of ILO</b></h3><p>Many in the NDIS have considered <b><a href="https://empathiagroup.com.au/individualised-living-options/">Individualised Living Options</a> </b><span style="font-weight: 400;">(ILO) as the source of community integration that the scheme initially promised. </span></p><p><span style="font-weight: 400;">In most cases, ILO creates a fundamental re-orienting of the accommodation arrangement where people can truly experience their communities and social inclusion.</span></p><p><span style="font-weight: 400;">However, the data continues to demonstrate that ILO still represents a vanishingly small amount of accommodation payments, and the growth is limited in both a rate and absolute sense. In this article, we review the ILO market from a data-driven perspective.</span></p><h3> </h3><h3><strong>The data doesn&#8217;t lie</strong></h3><p><span style="font-weight: 400;">Firstly, with our focus on the stats, we accumulated several NDIS data sets that finally allow us to understand the trajectory of the ILO market. </span></p><p><span style="font-weight: 400;">In a previous article, we calculated what full-scale ILO maturity would look like if we achieved Western Australian (WA) rates nationally. However, it appears that it will take some time before we get there.</span></p><p><span style="font-weight: 400;">ILO Exploration and Design grew from 522 participants purchasing in December 2021, to 684 in March 2022. This is a 31% growth rate, which when annualised is quite impressive. </span></p><p><span style="font-weight: 400;">However, this Exploration and Design activity only translated to an additional 41 customers purchasing the ILO support model between quarters (9%, 473 to 514). This number is quite concerning as it is indicative of a relatively high Exploration and Design failure rate. </span></p><p><span style="font-weight: 400;">At Empathia Group, we&#8217;ve been concerned about this issue since the original inception of ILO. At this growth rate, it will take a further 6 years before we achieve the WA rate nationally. </span></p><p><span style="font-weight: 400;">Of course, the scheme would need to maintain this rate at scale, which doesn’t appear possible given the limited host and co-resident sourcing infrastructure in the sector. We believe that significant interventions in this space will be needed to achieve ILO saturation consistent with customer demand.</span></p><p><span style="font-weight: 400;">It should be noted that the NDIS data sets are annualised, so we can’t make inferences about the success rate between quarters and what the average Exploration and Design time is. New South Wales (NSW) has experienced significant growth in this support area, with WA also enjoying substantial increases. </span></p><p><span style="font-weight: 400;">The “success” metric is highly confounded, but we’re attempting to capture “how much of last quarter’s Exploration and Design activity translated into actual increases in support model purchases”. This will be heavily affected by the timing in the annualised data (some people may still be using the same Exploration and Design package across quarters), and support model increases originated in prior quarters. Interestingly, WA continues to maintain the highest “success rate”, which likely reflects their existing host and co-resident matching infrastructure, which we believe is a major limiting factor in newer markets. </span></p><p><span style="font-weight: 400;">WA experienced the most significant growth in an absolute sense, but this reflected a lower rate of growth given the considerable denominator. However, given that WA accounts for only 10% of the national accommodation spending, they still have a decisive edge in the ILO market. This difference demonstrates a real demand for ILO products, and they simply aren’t provided at the scale required for the national market.</span></p><p><span style="font-weight: 400;">We find it concerning that WA contributed 45% to the ILO Support Model growth between quarters, with exceedingly marginal amounts found in the populous states nationally.</span></p><p><span style="font-weight: 400;">Our view is that this data demonstrates that ILO isn’t growing at a rate that can deliver this brilliant solution to community living at the rate customers expect. </span></p><p><span style="font-weight: 400;">As far as we can tell, there isn’t much difference between the WA folk and the rest of Australians (although some in WA will dispute this point), and the higher saturation in WA reflects genuine demand for what we consider to be an outstanding product.</span></p><p><span style="font-weight: 400;">Queensland (QLD) and NSW need to up their game when it comes to ILO, even though Victoria (VIC) and South Australia (SA) aren’t performing much better. </span></p><h3> </h3><h3><strong>Incredible client outcomes are possible</strong></h3><p><span style="font-weight: 400;">In the coming weeks, we will provide you with two brilliant ILO success stories from a small but exceptional NSW provider. Indeed, while this provider is relatively small, their dynamic team managed two extremely successful ILO implementations that have created excellent social and community improvements and delivered genuine improvements to quality-of-life outcomes for these customers.</span></p><p><span style="font-weight: 400;">This provider did not possess extensive infrastructure and was still able to deploy this product using a combination based on a solid commitment to their customers and a willingness to learn. We are yet to identify major barriers to ILO other than the will of providers. </span></p><p><span style="font-weight: 400;">An important detail to note in these cases is that both models utilised the “co-resident” model, which we initially expected to have lower uptake than the “host” model. While the current data format doesn’t allow us to identify different purchase patterns, we aim to generate our own data to inform you about the ILO models&#8217; relative success rates.</span></p><h3> </h3><h3><strong>The key takeaway</strong></h3><p><span style="font-weight: 400;">We find it interesting that despite a worsening environment for Supported Independent Living (SIL) providers, our modelling and work with customers demonstrates that ILO is viable for providers and attractive for both hosts and co-residents. </span></p><p><span style="font-weight: 400;">Indeed, ILO can give providers access to scaling effects that are excluded under the unit rate-based attendant care models. Given that 1. ILO works 2. Customers want it, and 3. <p id="viewer-62vq5" class="mm8Nw _1j-51 iWv3d _1FoOD _3M0Fe aujbK iWv3d public-DraftStyleDefault-block-depth0 fixed-tab-size public-DraftStyleDefault-text-ltr"><span class="_2PHJq public-DraftStyleDefault-ltr">The WA experience provides insight into how ILO might mature across Australia. In WA, ILO accounted for 5.36% of all accommodation based committed supports (SIL and ILO), while across the rest of Australia, it accounted for 0.23%. This extreme gap is accounted for by the fact that the WA ILO model is relatively mature – what happens if the rest of Australia achieves ILO maturity?</span></p>
<p id="viewer-4v558" class="mm8Nw _1j-51 iWv3d _1FoOD _3M0Fe aujbK iWv3d public-DraftStyleDefault-block-depth0 fixed-tab-size public-DraftStyleDefault-text-ltr"><span class="_2PHJq public-DraftStyleDefault-ltr">If the rest of the market catches up with ILO, then the implied maturity size of ILO is ~$310,000,000 annually. This is quite a substantial dent to put in the already shrinking SIL market. Indeed, our reading of the NDIS actuarial review suggests that ILO maturity isn’t accounted for in their SIL placement trajectory. While full ILO deployment would displace an additional ~900 potential SIL participants, it is challenging to model ILO entry into the future. However, we know that ILO is likely to impact the projected ~1200 SIL clients entering each year.</span></p>
<p id="viewer-95u04" class="mm8Nw _1j-51 iWv3d _1FoOD _3M0Fe aujbK iWv3d public-DraftStyleDefault-block-depth0 fixed-tab-size public-DraftStyleDefault-text-ltr"><span class="_2PHJq public-DraftStyleDefault-ltr">For the rest of Australia to catch up with WA, ILO would serve approximately 3000 customers, up from the current base of just 196 people using ILO support model payments outside of WA. Perhaps more curiously, ILO Exploration and Design was used by just 522 people in Australia last year, suggesting that the market must make deliberate efforts to catch up with our western counterparts.</span></p>
<p id="viewer-428af" class="mm8Nw _1j-51 iWv3d _1FoOD _3M0Fe aujbK iWv3d public-DraftStyleDefault-block-depth0 fixed-tab-size public-DraftStyleDefault-text-ltr"><span class="_2PHJq public-DraftStyleDefault-ltr">The ILO exploration model comprised average payments of $2,000 per participant. This equates to roughly 20 hours of coordination effort to complete the ILO exploration forms and identify the appropriate supports for a customer. This number seems reasonable for Empathia Group based on our ILO service model design.</span></p>
<p id="viewer-3shsk" class="mm8Nw _1j-51 iWv3d _1FoOD _3M0Fe aujbK iWv3d public-DraftStyleDefault-block-depth0 fixed-tab-size public-DraftStyleDefault-text-ltr"><span class="_2PHJq public-DraftStyleDefault-ltr">It does appear that the agency is making a concerted effort to kick start ILO. For example, in WA, the ratio of Stage 1 customers to stage 2 customers is just 28%, suggesting some degree of maturity. Nationally, this ratio is 225%, implying that ILO is in the ramp-up phase in the rest of Australia.</span></p>
<p id="viewer-2u49k" class="mm8Nw _1j-51 iWv3d _1FoOD _3M0Fe aujbK iWv3d public-DraftStyleDefault-block-depth0 fixed-tab-size public-DraftStyleDefault-text-ltr"><span class="_2PHJq public-DraftStyleDefault-ltr">The type of people accessing ILO support is broadly similar to other NDIS cohorts. As expected, 55% of ILO users are people living with an Intellectual Disability or Down Syndrome, consistent with their SIL user rate. </span></p>

<div class="">ABI typically attracts large support payments, which likely relates to specialised care services. An interesting finding is the psychosocial payment structure, which reflects much lower intensity. However, this data is probably skewed by a much lower presentation rate.</div>

For example, host arrangements were twice as common as co-residency arrangements but attracted 50% less funding than co-residency arrangements. This is potentially an artefact of high support rosters in a co-resident arrangement.</span></p>
<p id="viewer-71egs" class="mm8Nw _1j-51 iWv3d _1FoOD _3M0Fe aujbK iWv3d public-DraftStyleDefault-block-depth0 fixed-tab-size public-DraftStyleDefault-text-ltr"><span class="_2PHJq public-DraftStyleDefault-ltr">Our aim in writing this article was to reveal the mechanics of ILO to the rest of Australia. ILO is in full swing in WA, and the trends nationally suggest a ramp up elsewhere. Empathia Group’s view is that the WA cohort is not structurally different, and therefore many more national customers can enjoy the community-based living option offered by ILO.</span></p>

However, we believe this article demonstrates that the presentation rates and funding packages are sustainable for many NDIS organisations. Our view is that this represents an excellent opportunity for providers who have the core competencies to facilitate a more community based and natural living environment for their customers.</span></p>

For providers, a SIL program offers funding stability as the contact hours are high and customers often stay engaged for long periods, given it is their home. This begs the question &#8211; what will happen to these SIL providers if there is a major ILO uptake?</span></p><p id="viewer-1roit" class="mm8Nw _1j-51 iWv3d _1FoOD _3M0Fe aujbK iWv3d public-DraftStyleDefault-block-depth0 fixed-tab-size public-DraftStyleDefault-text-ltr"><span class="_2PHJq public-DraftStyleDefault-ltr">One way to gauge the potential ILO uptake is by looking at the size of the current SIL market and the numbers required to move across to ILO in order to balance the scheme. We’ve run the Monte Carlo simulation so you don’t have to. Key assumptions we’ve considered within the simulation include; customers coming in and out of ILO, the likely ratio and funding intensity of those customers in SIL and the likely ILO funding model. </span></p><p id="viewer-23gqk" class="mm8Nw _1j-51 iWv3d _1FoOD _3M0Fe aujbK iWv3d public-DraftStyleDefault-block-depth0 fixed-tab-size public-DraftStyleDefault-text-ltr"><span class="_2PHJq public-DraftStyleDefault-ltr">Our model suggests that 13,319 customers, or 57% of the SIL cohort will need to transition to ILO in order for the scheme to balance. While the agency has great intentions, and is doing a stellar job of managing an extremely complex organisation, let us consider what it would look like if the agency were able to achieve 50% of the modelled transitions within a 3 year period.</span></p><p id="viewer-9gk7c" class="mm8Nw _1j-51 iWv3d _1FoOD _3M0Fe aujbK iWv3d public-DraftStyleDefault-block-depth0 fixed-tab-size public-DraftStyleDefault-text-ltr"><span class="_2PHJq public-DraftStyleDefault-ltr">Based on the assumption that ILO participants would have resided in either a 1 to 1, 1 to 2 or 1 to 3 SIL setting, we have outlined below the implied hourly cost savings (note: this is based on the 230k benchmark price):</span></p><div data-hook="rcv-block10"> </div><div id="viewer-foagq" class="_2vd5k JP7h-"><div class="_3CWa- Dv9jC Dv9jC _3mymk"><div class="_2kEVY" tabindex="0" role="button" data-hook="imageViewer"><div class="_3WJnn _2Ybje"><img loading="lazy" decoding="async" class="OzAYt _3xs9_" src="https://static.wixstatic.com/media/ef8fd1_acbd72763ce34ba7ba94af1541ab3ffb~mv2.png/v1/fill/w_901,h_365,al_c,lg_1/ef8fd1_acbd72763ce34ba7ba94af1541ab3ffb~mv2.png" width="751" height="304" data-pin-url="https://www.empathiagroup.com/post/ilo-lift-off-a-provider-perspective-and-a-warning" data-pin-media="https://static.wixstatic.com/media/ef8fd1_acbd72763ce34ba7ba94af1541ab3ffb~mv2.png/v1/fit/w_751%2Ch_304%2Cal_c,enc_auto/file.png" /></div></div></div><p id="viewer-23b81" class="mm8Nw _1j-51 iWv3d _1FoOD _3M0Fe aujbK iWv3d public-DraftStyleDefault-block-depth0 fixed-tab-size public-DraftStyleDefault-text-ltr"><span class="_2PHJq public-DraftStyleDefault-ltr">As you can see, the agency has strong incentives to focus on higher intensity placements. Whether customers in 1:4 and 1:5 settings can be supported at the lower cost bands remains to be seen. However, it is important to remember that the decision to transition from SIL to ILO ultimately lies with the participant. While our simulation and figures outlined above utilise an average expected ILO funding figure, we expect these averages to be relatively consistent.</span></p><p id="viewer-914cd" class="mm8Nw _1j-51 iWv3d _1FoOD _3M0Fe aujbK iWv3d public-DraftStyleDefault-block-depth0 fixed-tab-size public-DraftStyleDefault-text-ltr"><span class="_2PHJq public-DraftStyleDefault-ltr">Considering the ILO model will skew to moderate intensity, this gives us a reasonable set of implications to consider. Let’s assume the average SIL provider loses 20% of its customers, and 50% of its incoming customer pool over the next 3 years. The first thing to note is that customer reduction behaves radically differently in SIL compared to other markets. The expenses incurred by SIL programs are almost perfectly inflexible, as client plans don’t change with changes to the support ratio. Vacancy risks within SIL have serious consequences for those organisations who find themselves with empty rooms. In the average 1:2 setting, a single customer leaving will result in a program loss of $93,000 annually. This issue is likely to be exacerbated by a significant reduction in new customers to SIL as the intensity of the ILO rollout increases. </span></p><p id="viewer-bijfh" class="mm8Nw _1j-51 iWv3d _1FoOD _3M0Fe aujbK iWv3d public-DraftStyleDefault-block-depth0 fixed-tab-size public-DraftStyleDefault-text-ltr"><span class="_2PHJq public-DraftStyleDefault-ltr">If we look at the average cash reserves of our SIL Provider Index then the situation gets much worse. Many of our organisations do not have the assets to survive an unmanaged SIL to ILO transition. Curiously, several scenarios were provided that allow us to form a view on how the model might be priced and what the implications may be for customers, providers and ILO teams.</span></p><p id="viewer-4blno" class="mm8Nw _1j-51 iWv3d _1FoOD _3M0Fe aujbK iWv3d public-DraftStyleDefault-block-depth0 fixed-tab-size public-DraftStyleDefault-text-ltr"><span class="_2PHJq public-DraftStyleDefault-ltr">The scenario of Tori in the consultation paper gives us a clear view on how much time is spent inside and outside of placement. Using scenario analysis we can make assumptions about what the equivalent SIL funding might have been. At the 1 on 1 rate, Tori’s support would cost around $410,000 a year, including public holidays and sleepover costs. The scenario suggests that the NDIS prices these supports at $115,000. If we consider the time in placement, the SIL model provides an effective hourly rate of $73.01 (including some assumptions about sleep over disturbance) and the ILO model provides an effective hourly rate of $20.41 &#8211; a 72% discount.</span></p><p id="viewer-e6q2c" class="mm8Nw _1j-51 iWv3d _1FoOD _3M0Fe aujbK iWv3d public-DraftStyleDefault-block-depth0 fixed-tab-size public-DraftStyleDefault-text-ltr"><span class="_2PHJq public-DraftStyleDefault-ltr">This raises several key questions about how an ILO setting might differ from the current SIL model. Is the NDIS expecting far less support in ILO settings? Or is there a view that ILO support is fundamentally different, and less cost intensive? If we assume the NDIS price the support in the same fashion, then an ILO setting would provide, on average, 30 hours of support a week, or about 28% of the support of a regular SIL placement. This presents us with a fundamental question about ILO &#8211; Does ILO provide considerably less support than SIL, or is ILO paid at a drastically lower hourly rate? </span></p><h4 id="viewer-8gga6" class="mm8Nw _1j-51 iWv3d _1FoOD _3M0Fe aujbK iWv3d public-DraftStyleDefault-block-depth0 fixed-tab-size public-DraftStyleDefault-text-ltr"><span class="_2PHJq public-DraftStyleDefault-ltr">Regardless of the above, it is our view that ILO may still be in the best interest of the person being supported. They are able to live in a way that is as close to regular life as possible, with support only being provided as needed.</span></h4><p class="mm8Nw _1j-51 iWv3d _1FoOD _3M0Fe aujbK iWv3d public-DraftStyleDefault-block-depth0 fixed-tab-size public-DraftStyleDefault-text-ltr"><span class="_2PHJq public-DraftStyleDefault-ltr"> Indeed, this aligns nicely with both the concepts of active support and person-centredness. Of course, we are staunch advocates for such an approach, however the implications for support professionals and organisations need to be considered.</span></p><p id="viewer-37kqb" class="mm8Nw _1j-51 iWv3d _1FoOD _3M0Fe aujbK iWv3d public-DraftStyleDefault-block-depth0 fixed-tab-size public-DraftStyleDefault-text-ltr"><span class="_2PHJq public-DraftStyleDefault-ltr">Firstly, Empathia Group are strong advocates for the professionalising of support work. It is a valuable and complex profession that makes a genuine difference in people’s lives. However, the ILO model implies that either the support provided under SIL is cheap to replicate, or that SIL supports are far too intensive. We are unable to reconcile either perspective.</span></p><p id="viewer-5kr02" class="mm8Nw _1j-51 iWv3d _1FoOD _3M0Fe aujbK iWv3d public-DraftStyleDefault-block-depth0 fixed-tab-size public-DraftStyleDefault-text-ltr"><span class="_2PHJq public-DraftStyleDefault-ltr">For organisations there are serious repercussions regarding how work is recognised and rewarded. The funding model proposes a radical rate reduction from SIL, which implies that support must be far less intensive. However, the implied hours are far too low for people to receive support in a fashion remotely similar to SIL. Indeed, if people are funded for only reasonable and necessary supports, then someone is going to foot the bill for the support gap. I suspect this will take the form of voluntary support from ILO host teams, who are likely more than happy to do it. </span></p><p id="viewer-ab7bt" class="mm8Nw _1j-51 iWv3d _1FoOD _3M0Fe aujbK iWv3d public-DraftStyleDefault-block-depth0 fixed-tab-size public-DraftStyleDefault-text-ltr"><span class="_2PHJq public-DraftStyleDefault-ltr">If we take the view that ILO supports are fundamentally different, and are truly “informal” then the consequences are considerable. For participants who access ILO, the supports they receive will undeniably replace those which would have previously been provided by paid support workers under the SIL model. The NFP sector has learned that volunteer work must not replace work which can be performed by professionals, as demonstrated by a number of precedents within the FairWork Commission. </span></p><p id="viewer-5014p" class="mm8Nw _1j-51 iWv3d _1FoOD _3M0Fe aujbK iWv3d public-DraftStyleDefault-block-depth0 fixed-tab-size public-DraftStyleDefault-text-ltr"><span class="_2PHJq public-DraftStyleDefault-ltr">At any rate, the ILO proposal is a “de-formalising” of support work, with implications in both directions. As we saw in the early days of SIL with weekly “block funding”, the true cost of support tends to benefit one party. Unfortunately, informal systems tend to incentivise intense customer selection, where providers only seek to support customers who can be assisted within the constraints of the informal price model.</span></p><p id="viewer-fu4pp" class="mm8Nw _1j-51 iWv3d _1FoOD _3M0Fe aujbK iWv3d public-DraftStyleDefault-block-depth0 fixed-tab-size public-DraftStyleDefault-text-ltr"><span class="_2PHJq public-DraftStyleDefault-ltr">ILO represents a radical departure from formalised SIL funding and will create a support surplus, or support gaps that will need to be accommodated by someone. </span></p><p id="viewer-2lkmj" class="mm8Nw _1j-51 iWv3d _1FoOD _3M0Fe aujbK iWv3d public-DraftStyleDefault-block-depth0 fixed-tab-size public-DraftStyleDefault-text-ltr"><span class="_2PHJq public-DraftStyleDefault-ltr">We would love to keep this dialogue going, feel free to <a href="http://www.empathiagroup.com.au/contact">contact us</a> to discuss.</span></p>								</div>

The Agency’s constant messaging about the increasing average cost of SIL is a strong indicator of the trepidation they have about the ongoing sustainability of the scheme. </span></p><p id="viewer-7n1ce" class="mm8Nw _1j-51 iWv3d _1FoOD _3M0Fe aujbK iWv3d public-DraftStyleDefault-block-depth0 fixed-tab-size public-DraftStyleDefault-text-ltr"><span class="_2PHJq public-DraftStyleDefault-ltr">The recent consultation paper <em>“An Ordinary Life at Home”</em>, released in September 2021, provides a strong indication about the direction the agency wants to take. Their vision of “<em>an Australia where people with disability live ordinary lives in ordinary homes and are included in their communities</em>” (NDIA: Consolation Paper: An Ordinary Life at Home, September 2021) is a bold and inspirational position. The paper emphasises Individualised Living Options (ILO) as its pivotal way to make the change towards its newly articulated vision. It even goes so far as to raise some serious concerns with SIL in its current form and in its inability to create an ordinary life for people with a disability, including the highlighting of research that shows that people with a disability in SIL are at a higher risk of abuse and neglect; inappropriate placements; workforce issues; cultural issues; and isolation. Is this a clue to perhaps the demise of SIL as we know it?</span></p>								</div>

ILO could be the answer. The Agency’s hypothetical example of a client moving from SIL to ILO is used to highlight the potential cost saving of 18.5% that would be created. </span></p><p id="viewer-2npko" class="mm8Nw _1j-51 iWv3d _1FoOD _3M0Fe aujbK iWv3d public-DraftStyleDefault-block-depth0 fixed-tab-size public-DraftStyleDefault-text-ltr"><span class="_2PHJq public-DraftStyleDefault-ltr">In my view, ILO would have a positive impact on the lives of people with disability as they are afforded more choice, control and improvements in living arrangements, not to mention the impact on the sustainability of the scheme now and into the future. </span></p><p id="viewer-f1je8" class="mm8Nw _1j-51 iWv3d _1FoOD _3M0Fe aujbK iWv3d public-DraftStyleDefault-block-depth0 fixed-tab-size public-DraftStyleDefault-text-ltr"><span class="_2PHJq public-DraftStyleDefault-ltr">Market research suggests that there are insufficient ILO providers to meet current demand. This gap in the market is only going to widen as the Agency switches it’s focus from SIL to ILO.
